Belt size is the waist measurement in cm, taken from the buckle to the middle hole. Choosing by trouser size lands you on the middle hole.
Measure a belt you own. Lay the belt flat and measure from the buckle prong to the hole you use.
Or go by your trousers. If you wear 32" trousers, a 90 cm belt will fit.
